Health Forms and Documents
The Foundation
for Healthy Communities in New Hampshire
created this new tool for New Hampshire health care providers to easily
find translated health forms and documents. They recommend that the translated
forms in 10 languages be used with their English counterparts to ensure that all
parties are aware of what is being communicated. Each translated form also is
posted in English. An unique facet of this tool is that the documents are posted, when possible, in a Word format in order to allow for easy customizing.
