This Spanish-language publication from the
National Institute on Aging helps older Hispanic consumers communicate effectively with their doctor and other health care providers. It also assists in helping them choose a doctor, prepare for an appointment, work with an interpreter, discuss sensitive health issues and find additional health information in Spanish. Provides consumer information specifically in the languages of Chinese, Hmong, Korean, Cambodian/Khmer, Thai, Vietnamese and Laotian. Start your information search either by
topic or language. This is a joint initiative of the South Cove Community Health Center and Tufts University Health Sciences Library, both located in New England.

Translated Health Forms and Documents
The
Foundation for Healthy Communities in New Hampshire created this new tool for New Hampshire health care providers to easily find translated health forms and documents. They recommend that the translated forms in 10 languages be used with their English counterparts to ensure that all parties are aware of what is being communicated. Each translated form also is posted in English. An unique facet of this tool is that the documents are posted, when possible, in a Word format in order to allow for easy customizing.
